Measure Dx
Diagnostic errors are major contributors to patient harm. Approaches to identify and analyze these events continue to be in development, but several show potential for use in operational settings.
Measure Dx, a new patient safety resource released by the Agency for Healthcare Research and Quality (AHRQ), helps health system leaders, quality and safety professionals, and clinicians advance efforts to improve diagnostic safety and reduce patient harm.
The tool provides teams with practical guidance and strategies to detect and learn from diagnostic safety events in their organizations.
It includes a checklist to gauge readiness for implementation, measurement strategies, and recommendations for analyzing data and translating findings into improvement.
